    Eligible applicants must be non profit organizations located in Pennsylvania, Connecticut, New Jersey, or New York.

    Deadline Details

    Application materials must be submitted by November 26, annually. Applicants must contact the Foundation for an application form. Applicants should submit the following:

    • Listing of additional sources and amount of support
    • Copy of current year's organizational budget and/or project budget
    • Listing of board of directors, trustees, officers and other key people and their affiliations
    • How project's results will be evaluated or measured
    • Copy of IRS Determination Letter
    • Brief history of organization and description of its mission
    • Populations served
    • Statement of problem project will address
    • Qualifications of key personnel
    • Timetable for implementation and evaluation of project
    • Copy of most recent Annual Report/Audited Financial Statement/990
    • Detailed description of project and amount of funding requested

    Award Details

    Awards vary based on scope and size of project. Awards are up to $50,000. Most awards are less than $5,000.
